Disorders of self-control such alcohol abuse, drug dependence, etc. typically have high relapse rates despite energetic treatment efforts.   Often the individual does well during the early phases of treatment when external sources of control are salient.  However, dependence on external sources of control to maintain sobriety makes one vulnerable when treatment ends - for whatever reason - and the source of control is removed.
